Lecture 3 - Infratemporal Fossa Flashcards
bony boundaries
zygomatic arch
mandible ramus
condyle (TMJ)
lateral pterygoid plate (medial boundary)
temporomandibular joint (TMJ)
synovial joints
-hyaline cartilage covers articular surfaces
-joint capsule (fibrous CT) connects bones
-synovial membrane line non-weighbearing surfaces
-articular disc dividing joint space
>disc divides joint space to upper and lower
movement
-hinge: elevation and depression
-gliding: protraction and retraction
-side to side: use muscles on alternating sides for grinding
dislocation
-movement of chondyle past the articular tubercle (keeps head of mandible from sliding too far forward when open)
muscles of mastication
first branchial arch - innervated by CN V3
masseter - elevation and protraction
temporalis - elevation and retraction
lateral pterygoid - depression and protraction
medial pterygoid - elevation and protraction
- pterygoids also move mandible from side to side
- lesion cortical input to motor V3 = some weakness
- lesion motor V3 = loss of muscle function
- jaw deviates to weak side
maxillary artery
goes through infratemporal fossa and runs through the pterygomaxillary fissure
foramen ovale
middle meningeal artery goes through to supply the meninges
V3
venous drainage
pterygoid plexus
-maxillary vein
-retromandibular vein
goes to ext. and int. jugular vein
mandibular division of V3
starts in cranial cavity
trigeminal/semilunar ganglion
-V1 and V2 are sensory (ophthalmic and maxillary)
-V3 (mandibular) is motor, goes through foramen ovale
Nerves of the infratemporal fossa
Sensory -ariculotemporal *** also has parasymp (from 5->9 parotid) -inf. alveolar -lingual *** also has parasymp + taste for ant. 2/3 tongue -temporal -buccal Motor -inf alveolar -N. mylohyoid (nerve): ant. belly digastric and mylohyoid -temporalis -medial pterygoid -lateral pterygoid -masseteric - last four innervate muscles its named for BOTH -mental
cranial parasympathetics
parotid gland
- innervated by preganglionic parasympathetics from CN 9 to the otic ganglion
- parasymp “hitckhike” with auriculotemporal nerve from V3
submandibular and sublingual glands
- chorda tympani nerve carries CN 7
- jumps to lingual nerve of CN 5
- synapse at submandibular ganglion and branch out
