Lecture 3 - Fracture and bone healing Flashcards
1
Q
Summarise what happens when a bone breaks (5 stages)
A
- Haematoma formation
- Acute inflammation - neutrophil first then m.phage
- Fibrocartilaginous callus formation
- Bony callus formation
- Bone remodelling
2
Q
Difference between centripetal and centrifugal blood supply to bone?
A
centripetal
- out to in, children
centrifugal
- in to out, adults
3
Q
Summarise bone healing
A
- Primary —> woven bone
- Secondary
- —> endochondral ossification
- —> intramembranous ossification
- —> woven bone
4
Q
How is framework for bone set down?
A
- fibroblasts
- first collagen type 3 then collagen type 1
5
Q
Difference between endochondral and intramembranous ossification
A
- endochondral, from cartilage
2. intramembranous, directly from mesenchyme
6
Q
Endochondral ossification
A
- mesenchymal cells —differentiate —-> chondroblasts
- chondroblasts —secrete—-> matrix —-> perichondrium
- chondroblasts —-buried in matrix —-> mature to chondrocytes
- Hypertrophy of middle chondrocytes
- –> some burst
- –> release of contents changes pH
- –> some burst
- this triggers calcification
- as cartilage calcifies chondrocytes begin to die —> nutrients cannot diffuse
7
Q
Intramembranous ossification
A
- mesenchymal stem cells — condense —- differentiate —-> osteogenic cell —-> osteoblasts
- Osteoblasts —- secrete —- uncalcified bone matrix —–> osteoid
- osteoid —- plus calcium —-> calcifies with help of alkaline phosphatase enzyme
- OB when surrounded by bone matrix —> osteocyte
- Lacunae form: hollowed out spaces
- Bony spicules from matrix fuse around BV, increased nutrient delivery
- Spongy bone remodelling
- —> osteocyte organsies themselves into osteons —-> compact bone!
8
Q
Compare intracapsular and extracapsular fracture
A
Intracapsular
- poor BS
- replacement
Extracapsular
- BS okay
- replacement not necessary
- fixation
9
Q
Describe bisphosphonates as treatment
A
- Analogues of pyrophosphate which is a molecule with decreases demineralisation of bone.
- Inhibits osteoclasts by reducing recruitment and promoting apoptosis
10
Q
Bisphosphonates types
A
- nitrogen containing (kills OB)
2. non nitrogen containing (stops OC)
