Lecture 17: Diarrhea Case Studies Flashcards
Why is the colon triangular?
Because of the taenia coli
Because they are crunched up into three points
What does volume of the diarrhea suggest?
Colonic process = small volume
Small bowel = large volume
Inflammatory process = blood
What is coryza?
Symptoms of a cold
Can ischemia skip?
Yes they can
Because there can be two independent vascular events
What is marker of crypt distortion?
BIFURCATION of the crypts
What protects against edema?
Albumin
What can a snake like colon be construed as?
Brownish
Use of laxatives
Specific laxatives that contain anthraquinone
Melanosis colon
If you see brown, what does that suggest?
LAXATIVE USE
Specifically anything containing anthraquinone
Fucks up brush border and allows shit to pass through
Lipofuscin pigment through the macrophages
What is the significance of anthraquinone?
The ingredient in the laxative that fucks up brush border and allows shit to pass through
Leads to melanosis coli
How do you know somebody has CMV caused of diarrhea?
Immunosuppressed patient
Nuclear inclusion
Cytoplasmic inclusion
CMV
How can you tell there is damage to the villous (or the absorptive component) of the small bowel?
There should be GOBLET CELLS Lack of goblet cells = villous damage
How is it possible to have a stool mOsm of 120? (needs to be 290)
Stool mOsm = 120 so it is not physiologically possible
Diluted by adding something to it
Fistula (bladder)
Psychiatric services, factitious
How did the stool go to 560 mOsm?
Because the bacteria cleaved many molecules while that shit was sitting in the sun
So get the diarrhea calculated ASAP
What is rice water stools suggest?
Vibrio cholera Cholera toxin (A and B) increase in cAMP which increases chloride secretion\
What is treatment for cholera?
Treatment = Gatorade
Give them electrolytes that are actively transported through small intestine so that water can follow the electrolytes
