Lola is a “happy little dog” that has been presented for a spay after having been fasted for 12 hours. Present a problem list for Lola:
- Hypotension
- Hypoventilation
- Hypothermia
- Painful procedure
- Recovery is painful
- Recovery excitation
- Behaviour – happy
What are the three aims of the surgery for Lola?
- Unconsciousness
- GOOD analgesia
- Muscle relaxation
What drug should be given as premedication for Lola?
ACP and opoid (methadone in this case)
What drug are we going to give to induce anaesthesia?
Propofol
What items should be gathered in preparation for anaesthesia?
- Cather – right size catheter, tape, disinfectant & clipper
- Flush with 0.9% NaCl
- Fluids and fluid set-up
- Drugs – induction + top ups + emergency drugs
- Monitoring – oesophageal stethoscope + other monitoring devices
- Intubation: 3 ETT, lidocaine spray, laryngoscope, cuff syringe, tie
- Anaesthetic machine
What sized endotracheal tube should be used for Lola? and what other tubes should be have on the table?
Use endotracheal tube size 8 for Lola - also think about having 7 and 9 - we want to go up one and down one on our table
What anesthetics can be used for the maintenance of anesthesia?
- Isoflurane and sevoflurane – gas anaesthetics that we can use
- Propofol – something that we can use if we did not have access to gas anaesthetic
When using isoflurane what settings should be used to keep the animal asleep?
1.5% to 2% - setting of Isoflurane that should be used
MAC value for isoflurane is 1.3% - this is the minimum level however it can go up to 2% with a surgical stimulus
During clipping a prepping Lola begins to wake up what steps could be taken?
Give a top up of propofol - need to give 1mg/kg of 1/10th of the induction dose.
Turning up the vaporizer would not work as it takes 10 minutes to get to the animals brain
What drugs can be given to reduce the amount of isoflurane that is required in a surgery?
- Methadone bolus
- Fentanyl bolus
- Medetomidine top up
What drugs can be given for post-operative pain relief?
- Lidocaine and Bupivacaine – should be given intra-operatively
- Methadone, Buprenorphine, Butorphanol, Tramadol – can be given for intraoperative analgesia
- Carprofen and meloxicam – NSIAD’s
