• the ETTO principle means that it is impossible to maximise efficiency and thoroughness at the same time.
• violation of certain (safety) rules happen for different reasons:
  >Example: routine violations (happen all the time, “everybody does this”)
  –> can even be intentional but not to harm, but to be more efficient, quickly

=> there are some rules one cannot possibly follow if they want to do their job
- so people just take the RISK
