Must be reapproved by a chief deputy or the fire chief prior to the end of each additional 30 day period.
Who must approve classes and activities while in a limited duty assignment?
Approval of his or her deputy chief.
When in a light-duty assignment, what are the restrictions with exercise?
Must be approved by the appropriate medical provider.
When taking IF time during a limited duty assignment, what are the hours specified to take for medical appointments etc.
Two hours for one medical appointment.
Three hours for two medical visits per day.
Must work a minimum of four hours per day when taking IF days.
Max I F days per week is five days.
What are the procedures in a limited duty assignment when a medical provider releases that individual?
Notify return to work section via telephone immediately.
Fill out and mail form 385 and 386 within 24 hours of the doctor visit to the return to work office.
A photo copy goes to the immediate supervisor.
Complete and sign the limited duty assignment agreement form 384 and submit it to the limited duty assignment supervisor.
