What is CLIA?
A
CLIA stand for Clinical Laboratory Improvement Amendment. The one runs an independent laboratory should be CLIA certified.
Urine analysis i.e. 81001, 81002, 81003, 81004 codes are exceptional to CLIA, but should be added with a modifier i.e. QW which means to waive off the CLIA#.
Place of Service 81 is for Independent Laboratory.

OA 23 means in Secondary’s EOB, claim got denied because Primary already paid more as compared to Secondary’s allowed amount.

A
NDC stand for National Drug Code. NDC are unique 11 identifiers for drugs, they help us to know manufacturer name, product details, quality, quantity, dosage, size and strength of the drug.
