Large Vessel Vasculitis Flashcards
What are thw 2 common features of all vasculitis regardless of size….
-Non specific symptoms of inflammation: fever, myalgia, weight loss, fatigue
-symltoms of organ ischemia ( depending on which vessel affectes)
What are the 2 main large vessel vasculitis?
Temporal (giant cell) arteritis
Takayasu arteritis
Which vessel is invloved in giant cell arteritis
Branches of carotid artery
Who is most commonly affected woth GC arteritis?
Older than 50 especially females
What are the clinical features of GC arteritis?
- unilateral headache (due to temporal branch)
-Visual disturbance (due to ophthalmic branch)
-Jaw claudication (painful chewing due to maxiliary
-Polymyalgia rheumatica (flue like symptoms with joint and muscle pain
How do you diagnose GC arteritis?
Elevated ESR (indicates inflammation aka not specific)
Segmental intimal fibrosis on biopsy (so -ve doesnt exclude disease)
What is the treatment of GC arteritis?
Corticosteroids
Complication of GC arteritis…
Blindness
Which vessel is affected in takayasu arteritis?
Arch of the aorta’s branching points
Clinical features of takayasu arteritis…
Neurological symptoms (headache, visual disturbance, numbness and tingling
Weak/absent pulse on upper extremities (pulseless disease)
Who is most commonly affected with takayasu arteritis?
Less than 50 yrs old especially asian females
Diagnosis of takayasu…
Elevated ESR
Treatment of takayasu…
Corticosteroids
Differences between GC and takayasu…
-GC affects older than 50, takayasu less than 50
- polymyalgia rheumatica in GC, pulseless disease in takayasu
-Branches of carotid in GC, aortic arch branch points in takayasu
