- Do you roughly know how much you WEIGHT?
- YOUR HEIGHT?
- (PT weight 125 and Height is 5’3. Which tells me pt is FIT and falls in healthy BMI range)
- Have you had any RECENT weight LOSS/ GAIN?
- Any changes on how your clothes have been fitting TIGHT or LOOSE?
- Any FEVERS,WEAKNESS or FATIGUE?

- Patients state of health is FIT.
- Patients state of awareness/ level of consciousness is AWAKE,ALERT,RESPONSIVE.
- Patients does not show any signs or has any complaints of distress like PAIN, ANXIETY/DEPRESSION, CARDIAC/PULMONARY PROBLEMS.
- Patient is present w. good HYGIENE AND GROOMING: no odors from body or breath.
- ALTHOUGH pt has a mask on pt shows appropriate eye contact and facial expressions are normal.
- Patients is seated in an UPRIGHT position and MOTOR activity is INTACT.

NOW we are going to begin with you vital sign portion of the exam:

- PULSE
- RR
Pulse math: 35x2= 70 BPM reg REG RATE, RHYTHM
RR math: 8x2= 16 reg REG RATE, RHYTHM (which was taken OBTRUSIVLY SO PT is not focused on their breathing) - FREE ARM OF CLOTHING.
4.palpate brachial artery.
5.Cuff fits comfy?? not new NP so 1 arm. - I will now take you SYSTOLIC pressure by palpating your RADIAL ARTERY/ inflating cuff.
- # + 30 units will help accurate BP
- BP
