Patient Identification
Confirm name & D.O.B by asking and cross-referencing with requisition and patient wristband if applicable
Requisition
Test order form, lists which tests are bring performed & used to determine which tubes to use
Label
Name, DOB, MR#, Date, Time, Initials (Full name if blood bank)
Patient Prep
Hold out arm, tourniquet, palpate to locate vein, remove tourniquet, apply alcohol swab, gather materials, re-tourniquet, begin draw
Angle of Insertion
30 degrees or less
Anchor Veins
Holding down vein below site of venipuncture to stabilize vein
Three Acceptable Veins
Median Cubital: ideal because stable, easy to locate, fewer nerve endings
Cephalic
Basilic
Tubes, Additives, Labs, Levels
Sterile: None, Micro?, None
Light Blue: Na Citrate, Coag, 90% full
SST: Silica/gel, Chem, 50% full
Red: Silica, Immuno, 50% full
Green: Heparin, Chem, 50% full
Lavender: EDTA, Hem, 50% full
Pink: EDTA, Blood bank, 50% full
Grey: Sodium Fluoride, Potassium Oxalate, Chem, 50% full
Needles
Straight, 21g, eclipse
Butterfly, 23g
Tourniquet
1 minute max, if on for 1min leave off for 2 minutes

Sterile, Light Blue, SST/Gold, Red, Green/Mint, Lavender/Pink, Grey, Yellow, Navy

Needle holder, self-explanatory
