Parallel Bars

A
  • Component: Balance and propriocpetion
  • How to use:
  • Manipulates the client’s oculomotor, vestibular, proprioception system
  • Can have the bars narrow/wide (in or out button) or high/low (up or down button)
  • Have parallel bars leveled at client’s wrist
  • HOWEVER, if client is unsteady, have the bars narrower
  • CAN challenge the client but have the bars wider
  • Be in the parallel bars with your client, facing in front of them
  • IF client is at risk of falling, have an assistant behind client
  • Exercises performed:
    1. Static balance exercises
    2. Reactive balance exercises
    3. Seated balance exercises

What are the three types of exercises client’s can perform on the parallel bars and describe what to do.

A

I. Static Balance Exercise
- Wide stance
- Narrow stance
- Tandem stance (heel to toe)
- Single leg stance
II. Reactive Balance Exercise: Apply external force to client and see how well your client can react by going back to their normal position
- NUDGE your clients from the front, back, and side to side
- Go through each stance as listed above
III. Seated Balance Exercise
- Tell client to scoot to the front of their chair (client’s back is off from the back support of chair)
- Position bars to about client’s shoulders height
- Can still do the same set of stances and reactive balance
- Weight shifting

How can you challenge clients and manipulate their systems on the parallel bars?

A
  • To CHALLENGE clients, tell them to gradually take one hand off the bar, and the other.
  • To MANIPULATE their balance systems, tell client to close one and to track your hand (side to side, up and down, rotational)

Biodex Balance Master

A
  • Component: Balance and propriocpetion
  • How to use:
  • Make sure force plate is secured and locked before getting on equipment
  • Position client’s feet centered on the force plate, hip-width apart
  • Turn on equipment by pressing the “ON” button. Then select “TRAINING” and select game. Input the client’s height
    • Left area of the screen shows how to manipulate the game, middle shows the game, right side is the static scale with score and time of game
  • IF NO NUMBERS are highlighted RED = Force plate is in static setting
  • IF ANY NUMBERS ARE HIGHLIGHTED, DO NOT START THE GAME and MAKE SURE YOU’RE STATIC
  • Hit “START” and fulfill the requirements of the game
  • When done using the equipment, press “STOP”, make sure the force plate is fully locked, then press “ON” button to turn off the system

How can you challenge clients and manipulate their systems on the Biodex Balance system?

A
  1. Proprioceptive system - Change the starting position or don’t hold onto the railing
  2. Vestibular system - Tilt or turn head while playing a game
  3. (Probably best to avoid this) Oculomotor system - Closing one eye

Kinetron

A
  • Component: FMS (gross motor skill - standing)
  • Goal: Client to reach weight bearing of their total body weight (different when seated vs. standing)
  • How to use:
  • Push the seat to the front by unlocking and locking it
  • Make sure seat and footplates are lowered all the way
  • Transfer the client onto the seat and put seabelt on client if neccessary
  • Unlock the seat again and tell clients to lift their feet before pushing the chair back and locking it
  • Adjust the footplates and seat so the clients hip and knee is at 90 degrees; make sure it is high enough so it won’t be obstruct by the floor
  • Adjust the resistance (loacated in the back); 90 = easiest , 10 = hardest
  • Tell client to push down with their feet = simulates weight bearing
  • Can have client stand by extending the arm rest
  • When done, make sure client is seated, lower foot plate and seat, push in the arm rest

Fine motor skills cart

A
  • Component: FMS (fine motor skills)
    • Activities performed:
      i. Shoulder ladder
  • Shoulder flexion - fingers go UP the ladder
  • Shoulder extension - finger go DOWN the ladder
  • Shoulder abduction - Client sits at a 90 degree angle to the equipment and fingers go UP the ladder
  • Shoulder adduction - Client sits at a 90 degree angle to the equipment and fingers go DOWN the ladder
  • Shoulder horizontal flexion - Assistant holds the ladder horizontally, with client sitting at 90 degrees anad move their finger FORWARDS
  • Shoulder horizontal extension - Assistant holds the ladder horizontally, with client sitting at 90 degrees anad move their finger BACKWARDS
    ii. Graded ROM Arc
  • Instruct the client to grasp/hold onto one ring
    iii. Cone stacking - For grasping, grasping, pronation, and supination
  • Color coordination of cones
  • Stack cones to higher height
  • Changing the coordination and location of where you are stacking the cones

A

How to use: 1-3 sets of 8-12 repetitions
- Make sure client is seated properly and safely
- Ankles are placed in the foot support where feet don’t touch when dorsiflexing and low enough that pressure is off the calf muscle; Older machines allow change by twisting knob, new machines don’t
- 8 exercises (knee flexion/extension, shoulder flexion/extension, shoulder horizontal flexion/extension, trunk flexion/extension)
