Depending on the pts condition, fitness level and conditions / problems they have, an appropriate sport will be selected and certain classes of sports are included and excluded.

Step 1: Medical history - Previous disqualification - Detection of previous or current issues - Viral illness - Sickle cell trait - Hospitalizations or surgeries - Medications or allergies - Special equipment - Demographic data - Immunizations/tetanus - Weight changes - Anaphylactic reactions - Concussion history - Dizziness or collapse with exertion - Family history - Asthma - Organ loss (paired organs) or dysfunction - Menstrual history - Contact lenses, dental appliances or other devices - Fractures, dislocations and other injuries - Previous heat illness or muscle cramps - Source of history Step 2: Physical exam - Height, weight, BMI - BP - Visual acuity - Skin, eyes, heart, abdomen, genitalia, musculoskeletal screening (90 second or Garrick eval) Step 3: Clearance - Decision: unrestricted, restricted/limited (participation to certain sports), clearance with proviso, deferred clearance/pending (proviso of further eval), disqualification/not cleared

In addenda, refer back to what is being is being changed or edited by using reference of date/time to the note above.
