Corticorelin (CRH or Acthrel)
CRH–corticotropin releasing hormone
Used rarely to distinguish Cushing’s from ectopic ACTH secretion

Adverse effects of Adrenal steroids

Aminoglutethimide (Cytadren)
Inhibits conversion of cholesterol to pregnenolone thereby inhibiting all steroid synthesis
Used in cushing syndrome and breast cancer and prostate cancer not responding to other therapy
Causes adrenal insufficiency and these steroids must be added
Ketoconazole (Nizoral)
Inhibits steroidgenesis at higher doses
Inhibits 17alpha-hydroxylase to decrease cortisol, androgens and estrogens
First-line treatment of Cushing’s disease before surgery
Mitotane (Lysodren)
Adrenocorticolytis–causes adrenocortical atrophy, binds mitochondrial proteins and inhibits synthesis of corticosteroids
Used in primary adrenal carcinoma
Treats Cushings
Causes severe GI distress
