Osteoarthritis of the knee
> 50 years old, overweight
Pain, intermittent swelling, crepitus, restricted movement
Infrapatellar bursitis
AKA Clergyman’s knee
Associated with kneeling
Prepatellar bursitis
AKA Housemaid’s knee
Associated with more upright kneeling
Anterior cruciate ligament
Caused by twisting of the knee - ‘popping’ noise may have been heard
Rapid onset of knee effusion
Positive draw test
Posterior cruciate ligament
Caused by anterior force applied to the proximal tibia
Collateral ligament
Tenderness over the affected ligament
Knee effusion may be seen
Meniscal lesion
Caused by twisting of the knee
Locking and giving-way are common feature
Tender joint line
Baker’s cyst
Associated with arthritis, gout and minor trauma
Foucher’s sign - the increase in tension of the Baker’s cyst on extension of the knee
Transilluminable swelling in the posteromedial aspect of the knee
DVT needs to be considered because it can mimic a Baker’s cyst
DVT can co-exist with a Baker’s cyst - low threshold for ultrasound
