Where does the Q-Angle measure?
From the ASIS to Patella to the Tibia Tuberosity
What causes Patella Tracking?
Increased Q-Angle
Muscle tightness
Lax medial capsule
Hip muscle weakness
The larger the Q-Angle the worse the patella tracking is and an increase in ACL tearing
Where does the Common peroneal nerve extend?
Below the fibular head
Where does the Saphenous nerve extend?
Medial knee and leg
What does a Mild First Degree sprain in tale?
Incomplete stretch, minimal pain, minimal swelling, no loss of joint function, no instability
What does a Moderate Second Degree sprain in tale?
Few fibers torn, mod pain, some swelling, some loss of joint function, some loss of stability
What does a Severe Third Degree sprain in tale?
All fibers torn, profound pain, intense swelling, loss of joint integrity and function
How is the ACL torn from a contact injury?
Excessive anterior stress on the ligament
Combination of ER, Valgus stress and Internal tibial torsion
What does the ACL resist?
Resists anterior translation of tibia Hyperextension Valgus Varus Rotation
Hoe is the ACL torn from a non-contact injury?
Deceleration
Max phase of ACL injury
6-8 weeks
Strengthen quads within ranges and hamstrings
Avoid OKC in what range for Knee Ext of ACL Injury?
OKC Knee Ext: 40-0*
Avoid CKC in what range for Knee Ext of ACLE Injury?
CKC Knee Ext: 60-90*
What is the criteria in order to progress to the moderate protection phase in an ACL injury?
ROM from 0-120* FWB with normal gait mechanics Quad control Hams control Controlled pain and swelling 6 weeks post opt
Mod phase of ACL injury?
6-12 weeks
CKC of LEs
Quads and Hip strengthening is main goal
What is the criteria in order to progress to the min protection phase in an ACL injury?
Full ROM FWB with normal gait without brace Improved quads and hams strength Controlled pain and swelling Minimum 12-13 weeks post opt
Min phase of ACL injury?
Return to functional activities and education on body mechanics
What specials tests test for ligament stability of the knee?
Lachman’s and Anterior Drawer Test
Mild, Mod, Severe Instability mm?
Mild 5 mm or less
Mod 5-10 mm
Severe 10 mm
What is the mechanism of injury for the PCL
- posteriorly directed force on anterior aspect of flexed knee
- fall onto flexed knee
- hyperextension with foot planted
What do you need to remember when rehabbing the PCL?
Don’t activate the hams alone!!! They need to be activated along with the quads
ex: bridges & squats
