Keppler high risk part 2 Flashcards
An ultrasound can assess what two things for the child?
- Anatomic assessment
2. Growth assessment
What are involved in the fetal anatomic assessment
- Heart
- Brain
- Limbs
- Kidneys
- Face and palate
- Spine
- Placenta and cord
What does the biophysical profile look at?
- Fetal breathing
- Gross axial skeletal movements
- Motor Tone
- Amniotic fluid assessment
a biophysical score of 8-10 implies what?
No fetal asphyxia, repeat weekly
a biophysical score of 6 implies what?
suspected chronic asphyxia, if >36 weeks deliver, if less repeat test in 4-6 hours
a biophysical score of 4 and below implies what?
Strongly suspect asphyxia, if >36 weeks deliver, if not repeat after 4-6 hours for 120 minutes, persistent score <5 deliver regardless of GA
An amniocentesis looks at what 4 things?
- Genetic
- Fetal lung maturity
- Spectrophotmetry
- Assesses infection
How long is a NST and how long might it be extended to?
a NST is a 20 minute recording of fetal HR frequency. May extend to 40minutes if “non-reactive”
What elements does a NST look at?
- baseline frequency
- accelerations
- decelerations
- Variability
A reactive NST tells us what?
that there is a 99.8% likelihood the fetus will not die within next week from hypoxia
What are the two differences between continuous fetal monitoring and NST?
- continuous fetal monitoring is intrapartum
2. Real time assessment of fetus at a single point in time
