  1. He/she is a citizen of the ph or if a foreigner must prove that the country of which a subject or citizen permits filipino dentist to to practice within its territorial jurisdiction on the same basis as the subj, or citizens of such country
  2. Not been convicted of an offense involving moral turpitude
  3. Graduate of a recognized & legally constituted university with a degree of doctor of dental medicine
  4. he / she has completed the refresher course required after failing 3 consecutive licensure
