How is it diagnosed?

A

Clinical diagnosis

  • FHx
  • PMHx
  • Raised serum Uric acid
  • joint aspiration - MSU crystals
  • risk factors

How is it treated?

A

NSAIDS - Ibuprofen/Naproxen/Diclofenac
Colchicine - 500mcg BD/QID - (max 6mg over 3 days)
Prednisolone - 35mg OD 3-5days

Allopurinol 100mg OD - long term prevention

How does Psoriatic Arthritis present?

A

Common Patterns:

  • Asymmetrical oligoarthritis
  • Symmetrical polyarthritis
  • DIPJ predominant
  • Spondylitis
  • Arthritis mutilans

In patients with a history of Psoriasis

This is a type of Seronegative Arthritis

How is Psoriatic Arthritis treated?

A

Autoimmune - can use DMARDS

  • sulfasalazine
  • Methotrexate

NSAIDS for pain relief (or paracetamol/codiene)
Steroid injections
Physio

How is Pseudogout managed differently to Gout?

A

Just treat with NSAIDS - no effective long term prevention though reducing serum Calcium if hypercalcaemic may help (Alendronic acid - 10mg OD)
