What conditions may warrant anticoagulation?
A
AF Rheumatic heart disease Previous DVT/VTE Antiphospholipid syndrome Prosthetic heart valve Recurrent TIA
Do not anticoagulate people who have had stroke but are in sinus rhythm (unless there are other clues suggesting this is a venous thromboembolism or there is paroxysmal AF etc)

What is the usual reference range for INR?
A
2 to 3

How is warfarin dose decided?
A
Use INRstar programme - calculates weekly dose - divide between 7
Can be very varied - takes different amount in each patient to achieve target INR
Usually 5-10mg daily
