WHAT IS JAUNDICE?
Yellow discolouration of the sclera and skin
WHAT CAUSES THIS YELLOW DISCOLOURATION?
Due to hyperbilirubinaemia
AT WHAT LEVEL DO YOU CLASS HYPERBILIRUBINAEMIA?
> 50 micromole/l
HOW IS BILIRUBIN FORMED?
Destruction of red blood cells (specifically haem)
WHAT IS THE. NORMAL BILIRUBIN METABOLISM?
1) Bilirubin conjugated within the liver, making it water soluble
2) Excreted via bile into GI tract, majority of which edges red in faeces as urobilinogen and stercobilin.
3) Small percentage of urobilinogen is reabsorbed into bloodstream and excreted through the kidneys

1) Pre-hepatic
2) Intra-hepatic
3) Post-hepatic
WHAT IS PRE-HEPATIC JAUNDICE?
Excessive red cell breakdown which overwhelms the livers ability to conjugate bilirubin.
WHAT IS ACCUMULATED IN PRE-HEPATIC JAUNDICE?
Unconjugated bilirubin
WHAT IS INTRA-HEPATIC JAUNDICE?
Dysfunction of the hepatic cells leads to liver losing the ability to conjugate bilirubin
WHAT ACCUMULATES IN INTRA- HEPATIC JAUNDICE?
Both conjugated and unconjugated bilirubin
WHAT IS POST-HEPATIC JAUNDICE?
Obstruction of biliary drainage
WHAT ACCUMULATES IN POST-HEPATIC JAUNDICE?
Conjugated bilirubin
WHAT ARE EXAMPLES OF PRE-HEPATIC JAUNDICE?
- Haemolytic anaemia
WHAT ARE EXAMPLES OF INTRA-HEPATIC JAUNDICE?
- Alcoholic liver disease
- Viral hepatitis
- Hepatocellular carcinoma
- Iatrogenic - medication
WHAT ARE EXAMPLES OF POST-HEPATIC JAUNDICE?
- Gallstones
- Pancreatic cancer
- Abdominal mass
