what type of supervision is required for this skill?
direct
advantages of direct IV push
2
- smaller amounts of fluid (good for HF patients)
- rapid response
disadvantages of direct IV push
3
- no time to correct error
- speed shock
- death
What is direct IV push?
concentrated dose introduced directly into circulation
what is speed shock?
systemic reaction caused by toxic drug levels
S/S of speed shock
headache, syncope, flushing, chest tightness, irregular pulse, shock or cardiac arrest
what do you do if speed shock occurs?
stop injection immediately
keep syringe
assess VS
alert medical team
educate patient
ensure IV patency
document
number of increments is determined by?
small lines on syringe
calculating direct IV
total volume and time in seconds
- determines size and type of syringe
- time in seconds divided by # of increments
hyperkalemia shifting
steps
- check glucose
- prepare D50 and insulin in 1ml insulin syringe (NOT subcu syringe)
- administer D50 (over 5 min) first THEN seperate insulin
- check blood glucose
blood glucose check post hyperkalemia treatment
every 30 min x2 then after another hour
blood glucose check for hyperkalemia treatment if levels were 4-6
every 15 min x4 then after another hour
