IV Intro Flashcards
Why is safety important when adminstereing IVs?
-Are given directly into the blood stream
-Cannot retrieve if there is an error
-Can cause tissue damage and necrosis if IV infiltrates or extravasates
Before adminstering IV
CHECK IV SITE
Infiltration
NONVESICANT fluid in tissue
Infiltration Sx
swelling, discomfort, redness, pain
EXTRAVASATION
VESICANT fluid in tissue
What can extravasion cause?
necrosis, life threatening, surgical debridement, amputation

MAR

@ bedside
Min # of pt identifiers
2
Additive or Synergistic Effect
stronger reponse
Inactivation Effect
no reponse
Always separate incompatible or unknown compatibility drugs with a
saline flush (SAS)
SAS
Saline, agent, saline
Initial saline
confrms patency
Saline between meds
creates saline block
Final saline
flushes med and prevents catheter from occluding
PIC syringe size
3-5mL
Central line syringe size
ALWAYS 10mL (or larger)
The larger the syringe the lower
pressure
ALWAYS _____ all syringes!
label
NS and IVP look the same
Why dilute IVP med?
Decreases phlebitis to the vessel, required for caustic medications!
More accurate calculation and administration over allotted time
ROA of IVPB via pump
mL/hr
ROA of IVPB via gravity
gtts/min
ROA IVP delivered evenly over time
mL/15 seconds
Give - wait - give - wait - give….
IV Documentation IVPB
Sign MAR intials, date, time, sig
I/O for IVPB recorded once IV medicaton is completed.
Narrative note only needed if unexpected event occured.
