What are the different mechanisms for ischemic strokes?
Atrial fibrilation
Large/Medium Vessel Atherosclerosis
Small vessel arthrosclerosis
What are the signs of a Atrial fibrilation (cardioembolism)?
Acute stroke affecting multiple vascular territories
Large/medium vesel artherosclerosis
Small vessel atherosclerosis
What are symptoms of a middle cerebral artery stroke?
Contralateral (weakness, sensory loss, Visual field defect)
Gaze preference towards the side of the sroke
brocas abd Wernickes aphasia
left spatial neglect if on right parietal lobe
What will you find if you have ischemic anterior cerebral artery stroke?
•Contralateral leg weak and numb
What symptoms would you find in a ischemic posterior cerebral artery?
affects mostly the occipital lobe and will have mostly visual field defects?
What is the difference between a Cortical and a subcortical infarct?
Both will present with contrallateral weakness and numbness
Cortical infarct will have aphasia alsp
subcortical will not have aphasia
What can a posterior sirculation system infarction present?
Superior cerebellar, AICA, PICA, pontine perforators can all infarct.
Basilar occlusion can result in decrease LOC
- Vertigo
- Diplopia
- Nausea/vomiting
- Ataxia
- Weak
- Numb
Where do you get crossed signs?
always think brainstem
that is where things cross
- Crossed signs
- Right face weakness
- Facial nucleus and Corticobulbar tract
- Left body weakness
- Corticospinal tract
- Crossed in lower medulla
What are the symptoms of a basilar artery occlussion?
- This is a big deal
- Death
- Locked-in syndrome (only can blink eyes, nothing else)
- Severe disability
Retinal artery occlussion
Macula is bright and red because it receives blood supply from another artery
