Ischemic Heart Disease - INDY ppt Flashcards
Coronary artery disease definition
abnormal narrowing, ectasia, or occlusion or coronary vessels
Coronary heart disease definition
coronary artery disease that has become manifest as chronic stable angina, unstable angina, myocardial infarction, or heart failure
Ischemia definition
sub-normal myocardial blood flow
Metabolic syndrome
Any 3 of the following:
Hypertension (>130/85)
Abdominal obesity
-Waist circumference >40cm in men (>35cm in women)
HDL 150
Fasting plasma glucose >100
Associated with inflammation, coagulation abnormalities, progression to type 2 DM
What are some manifestations of coronary artery disease?
Chronic stable angina Unstable angina MI Ischemia cardiomyopathy (CHF) Sudden cardiac death Silent ischemia
What is angina pectoris?
- Visceral discomfort
- Diffuse & sub-sternal
- Accompanied by dyspnea, diaphoresis, nausea, light-headedness
- Provoked by physical exertion, emotional upset, heavy meal, working in cold temp
- Onset usually over several minutes
- Relieved by rest, NTG
Typical angina pectors?
Atypical?
Typical
- sub-sternal
- brought on by exertion
- relieved by rest of NTG
Atypical
-missing one or more of above
Atypical symtpoms - considered angina ‘equivalents’
Dyspnea Arm, jaw, or back pain Nausea Diaphoresis Fatigue Silent ischemia
Who typically experiences atypical symptoms?
Women
Elderly
Diabetics
High Risk Groups for Ischemic Heart Disease
What helps these groups?
Left main stenosis > 50%
3 vessel disease with reduced LV function
2 vessel disease involving proximal LAD
Multi-vessel disease in diabetics
Revascularization via CABG shown to improve survival
