What is IBS?
It is a dysfunction of large intestine without pathomorphologic changes in the intestinal wall
Etiology and Pathogenesis of IBS
Due to mental stress,psychic, psychosomatic factors, drug abuse
In Pathogenesis regulatory disturbances of diffuse endocrine system of large intestine
Clinical picture of IBS
-abdominal pain
-constipation
-diarrhea or their alteration
-meteriorism
-gastrocolic syndrome
All these manifestations are intensified by nervous factors and menstrual cycle in women
In most patients IBS combines with pathology of internal organs mainly of functional character : dyskinesia of the gallbladder,neurocirculatory dystonia, astenization of nervous system, algomenorrhea, dysmenorrheal in women and so on
Diagnosis of IBS
Based on clinical presentation
General condition of the patient is satisfactory
There can be palpatory tenderness of the peritoneal wall in the direction of the bowels and intestinal spasm.
Attention is paid to a psychic status of the patient
Proctosigmoidoscopy,fibre optic colonoscopy,irrigoradiography as differential diagnosis
Preparation for these procedures is necessary due to intestinal spasms and pain in patients with use of for trans, nervoflux
Treatment of IBS
-diet containing pectins,fibers, cereals (buckwheat,oatmeal),lots of liquid intake 1,5-2l a day
-according to the character of psychopathologic syndromes tranquilers, antidepressants, phylogenetic psychostimulants e.g eubiotics, polyenzymatic drugs , adsorbents of flatus(espumizan), spasmolytics, polyvitamins
Incase of gastric colic syndrome anticholinergics are used
Correction of dysbacteriosis and concomitant diseases as well as physiotherapy and sanatorium-spa treatment
