what is the classic pathologic change of iPD?
loss of the pigmented neurons from the substantia nigra
what are Lewy bodies?
eosinophilic cytoplasmic inclusion of alpha-synuclein (mostly) in neurons
what mutations are there for iPD?
SNCA, PARK2 (parkin) PARK7 (DJ-1) PINK1 (PTEN induced kinase), LRRK (dardarin)
environmental risk factors for iPD?
MPTP (heroin outbreak), pesticides, herbicides, heavy metals and solvents
in Dementia with Lewy Bodies, what are Lewy neurites?
abnormal alpha-synuclein rich neuronal processes in the gray matter
what are the two types of pathologies of Dementia with Lewy Bodies?
those that have lewy bodies in the brain stem, cerebral cortex: a rostral extension of the Parkinson’s disease
and those that have neocortical Lewy bodies and concomitant ALZ pathology
what is the usual presentation of dementia with lewy bodies?
visual hallucinations and REM sleep behavior disorders
what is the diagnosis of DwLB?
has to be clinical because the only definitive diagnosis is from pathology which is usually done at autopsy
in the ALZ with LB form, what is the current nomenclature?
mixed ALZ/Lewy Body disease
