Nutrition Care Process (NCP)
Nutritianlly-at-risk patients are thos that are showing signs of actual or potential for developing malnutrition:
- Lose/gain of >10% of usual body weight within 6 months
- 5% lose/gain in 1 month
- a weight of 20% over or under ideal body weight
- Presence of chronic disease or increased metaboluc requirements
Nutritianlly-at-risk patients are thos that are showing signs of altered diett or diet reschedules such as:
- Receiving total parenteral or enteral nutrition
- Recent surgery, illness and trauma
Including those not receiving food or notrition product (impaired ability to ingest or absorb food adequately for >7 days
Inadequate nutrition intake
