The HHPA requires members to notify the local medical officer of health as soon as possible of any reportable diseases defined in regulations
-Required to maintain confidentiality
-Consent NOT required
-Must be made in good faith
-Punishable by fine ($5,000)
Must contain: name & address, DOB, sex & date of symptom onset
