4 yo presents w/ Dennie Morgan pleats, xerosis and ichthyosis, and an itchy eczema on the flexor surfaces. What is the dg and tx?
Atopic dermatitis
Moisturizer, avoid irritants that likely caused the dermatitis
Pt presents w/ a fever and some warm tender plaques that feel boggy to palpation. Pt doesn’t recall any trauma recently. What’s the dg? Cause? Tx?
Cellulitis
D/t S aureus or Strep pyogenes
ATB (cephalosporin, macrolide, Clindamycin)
20-30 yo pt presents w/ tender nodules and plaques on their ankles, shins and knees. They aren’t ulcerating. They hurt and feel warm. Dg? Cause? Tx?
Erythema nodosum
Hypersensitivity response (infection, meds/OCs, herbs), often idiopathic
Anti-inflammatories and rest
Pt presents w/ scaly silver plaques on an erythematous base over their elbows, knees, and on their belly. When removed, the plaques reveal pin-point bleeding. The pt was tx’d for this before and it left weird stains, but it’s back now. Dg? Tx? Genetic predisposition?
