Palpation
Swelling / effusion Crepitus Tenderness Masses Fluctuance Temperature changes
Special Test
specific for each joint
Muscle Strength - Oxford Scale
0 - No contraction
1 - Visible / palpable muscle contraction but no movement
2 - Movement with gravity eliminated
3 - Movement against gravity only
4 - Movement against gravity with some resistance
5 - Movement against gravity with full resistance
Imaging - examples
Standard radiographs
MRI
PET
CT
Standard radiographs
Common
Assess for arthritis, fx, dislocations and others
Some soft tissue findings
MRI
soft tissue - internal derangement; herniated disc; occult fx
MRI athrogram
further evaluation of joints
PET
Tumors
Ca
CT
Bony detail
Esp. fx
Special Test - Examples
Arthrocentesis
NCS
Labs
Arthrocentesis
Remove fluid from joint for evaluation
ie - infection; gout
NCS
Done for numbness or other neurological concerns
Gives a rough location of neurological deficit
ie CTS, ulnar neuropathy
Labs
ESR
CRP
CBC - infection

Acetaminophen
common analgesic for mild to moderate pain
Often prescribed in OA and other ortho dz with pain
Do not exceed 4,000 mg in 24h
Can be hard on the liver
No anti-inflammatory properties
NSAID
Inhibit cyclooxygenase - the enzyme that converts archidonic acid to prostaglandins
More effective than acetaminophen, but more SE
Most inhibit COX-1 & COX-2
Safer GI profile
SE of NSAIDs
Can cause renal toxicity
Risk of GI bleed
Risk of CV problems
Examples of NSAIDs
Ibu Aleve Naproxen Diclofenac Asa Feldene
Examples of Pain medications
Hydrocodone
Oxycodone
Ultram
Muscle relaxer
Tx of muscle spasms often causing pain
Example of muscle relaxer
Flexeril
Skelaxin
Zanaflex
Glucosamine Chondroitin
Taken as supplements
Efficacy varies from pt to pt
Viscosupplementation
Medication injected into knee joint to decrease inflammation, decrease pain, provide nutrients to knee and lubricate knee
Tx of early arthritis
Examples of Viscosupplementation
Synvisc
Hyalgan
Orthovisc
Eufflexa
Steroid
Potent anti-inflammatory agent
Decreases inflammation and pain
Intra-articular or po
SE of steroid
Deterioration of bone / joint
Skin/fat atrophy
Osteoporosis
