Penicillin, b lactams, fluroquinolones, cephalosporins.
NSAIDs
allopurinol
furosemide
Rifampicin.
PPIs
other conditions causing acute interstitial nephritis
SLE , Sarcoidosis, Sjorgen syndrome - sss.
Hanta virus, staphylococci
featuresof interstital nephritis
Fever, rash, arthralgia
Eosinophilia
HTN
Investigations of interstital nephritis
Medication review
sterile pyuria - no bacs in urine but high WCC leukcytes or pus in urine.
white cell casts
Tx of interstitial nephritis
stop causative drug
glucocorticoid therapy if no improvements after stopping.
differential diagnosis: of intestitial nephrtis
Pre renal AKI –> low urine NA + high urine osmolality + improvement w fluid resus
Intrinsic aki -> ATN muddy brown cast + acute onset. Glomerulonephritis haemturia + RED cell casts.
Post-renal aki -> symps of BOO + bilateral hydronephrosis.
