How should you prepare for a person’s upcoming appointment?
•Tell the person the date and time, when appropriate
•Discuss what is going to happen at the visit
•Follow any instructions ordered to prepare for the visit
•Consider bringing items to keep the person occupied
•Encourage the person to wear loose and comfortable clothing in the event the HCP needs to do a physical exam
Make sure to have:
•Person’s insurance card
•Copy of current medication sheets or a list of medications
•HCP Encounter/Consult/Order Form
How to prepare yourself for the appointment?
•Read all paperwork before leaving so you can tell the HCP why the visit was needed when you are asked.
•Driving directions
•Money for parking, gas, food or drink
•Provider on-call information
•Family/Guardian information
•Name of the pharmacy, telephone number and directions
•A charged cell phone
What to remember during appointment visits?
•Check in w/ receptionist and state reason for visit
•Assist the person if needed
•Advocate, encourage participation and support abilities
When an appointment is done, what should you obtain?

What should a HCP order include?
•A person’s 5 rights
-Name
-Medication
-Dose
-Frequency
-Route
•Allergies
•HCP findings
•Special Instructions
•Target signs and symptoms, instructions and/or parameters for PRN medication
•An acceptable HCP signature
•Date of order
What should you do after a medical appointment?
•Make sure the pharmacy received the prescription
•Pick up new medications at the pharmacy or check on when the pharmacy will deliver the medication
•Bring back all forms, anyprescriptions, HCP orders, and the next appointment card to give to the appropriate person
•Transcribe all medication orders onto the medication administration sheet
°Post and Verify all orders
•Secure the medication
•Document the visit
•Communicate changes to all staff
What preparations need to be taken if you are bringing a person to the ER or hospital?
•Take the person’s
-current medication list
-insurance card
-HCP Encounter/Consult/Order form
•Be prepared to tell ER and/or hospital staff why you are bringing the person to the ER and/or hospital.
-Discuss with the HCP any scheduled medication the person may miss as a result of the ER and/or hospital visit.
•If you have any concerns about taking the person home (or to work/day program) after the visit, tell the ER and/or hospital staff and contact your Supervisor before leaving the hospital
•Medication reconciliation-comparing the hospital discharge orders to the orders prior to admission; any discrepancies must be clarified with the HCP
What does medication reconciliation ensure?
That a new medication ordered during a hospital stay is not omitted when the person returns home. It also ensures medication that was discontinued during a hospital stay is not administered when the person returns home.
What is needed for telephone orders?
•must be posted and verified twice
•may be administered without the HCP signature for 72 hours
•must be signed by the HCP within 72 hours
