intek tmj Flashcards
the bilaminar area is made up of what 2 lamella and what are they composed of
1) superior lamella = elastin
2) inferior lamella - collagen fibres
where to palpate medial and lateral pterygoid
medial is palpated at angle of mand
lateral is palpated i/o
blood supply to the tmj
- maxillary aa
- superficial temporal aa
what to ask for pain hx
- localisation of pain
- association with function
- use of analgesics
- age of patient
- hx of trauma
how to confirm anterior disc position in a patient with pain and no pain
no pain then take MRI
got pain then do arthroscopy
anchored disc - whats the range of motion
10-25mm which is the range of rotation
what is the difference between arthritis and arthrosis
arthritis has an inflammatory aetiology, eg RA, SLE
arthrosis is a degenerative change (usually no symptoms)
what are the aims of treating TMD
- Alleviate pain
- reduce joint loading
- reduce/ eliminate joint noises
- restore normal function
- prevent recurrence
how do tricyclic antidepressants work
- major neurotransmitter in the descending inhibitory system is 5-HT. manipulation of 5-HT causes a centrally acting analgesia and TCA blocks reuptake of 5 HT
what is baclofen
muscle relaxant
what is the wilkes classification used for
internal derangement of joint
what is a tmj eminectomy
is a surgical process to remove the articular eminence of the temporal bone, makes a non reducing disc into a reducing disc
what is a condylotomy, when is it done
it is when they cut off the condyle and interpose either an alloplastic material or muscle in between the two poles. done for tmj ankylosis
what are the contraindications of TMJ surgery
- local infective process
- severe immunocomrpomise
- severe coexistent disease (ASA III)
what are some complications of TMJ surgery
- facial weakness
- scarring
- persistent symptoms
- pain
