drugs and therapeutics 1 Flashcards
what drug requires serology testing before given to those with liver disease
prednisolone, because it can cause hep carriers to become active hepatitis again
what drugs to avoid in liver disease
- aspirin
- nsaids
- paracetamol
- benzos
- azoles
- clindamycin
azoles should be avoided in which group of patients
liver disease! all hepatotoxic
amide LA caution is which group of patients
liver disease
what drugs should have decreased dose in liver disease patients
- metro
- carbamazepine
- erythro
- clarithro
what AB is safe to give in patients with liver disease
beta lactams as these are metabolised by renal so excreted in urine
what drugs to avoid in renal disease
- aspirin
- nsaid
- cox 2 inhibitor
- tetracyclines
- aminoglycosides
- sulphonamides
- ciprofloxacin
what drugs to decrease dose in renal disease
- vancomycin
- cephalosporins
- acyclovir
what AB are safe for patients with renal disease
macrolides, metronidazole, clindamycin
what analgesic safe for patient with renal disease
- paracetamol
- LA
in geriatrics, a lot of them take statins. what drugs should we be careful of prescribing and what are their side effectts
1) metro - ddi with simvastatin, increases risk of nerve damage
2) antifungals - will increase the blood levels of atorvastatin and hence increase risk of liver damage and rhabdo
3) macrolides - macrolides inhibit cyp3a4 and cause increased blood levels of statins, increasing risk of myopathy and rhabdo
what drugs to caution in geriatrics
- sedatives
- hypnotics
- narcotics
- corticosteroids - fluid retention, severe gastritis, blood sugar goes up, plus these old folks are alrdy osteoporotic so corticosteroids long term will inhibit osteoclasts
what drugs cant take with milk
- tetracycline - absorption decreases a lot
- quinolones - calcium binds to fluoroquinolones and make them ineffective
what is the interactin of ibuprofen and warfarin
taking ibuprofen increases the chance of bleeding because both are 99% protein bound
concomitant administration reduces warfarin binding to 97% so got lots of free warfarin to increase bleedin
what are some AB which are liver enzyme inhibitors
- erythro, clarithro
- cipro
- azole antifungals
- metronidazole
