Suprane/Desflurane

  • Metabolism
  • Physiologic effects (cardiovascular, respiratory, etc)
  • Characteristics/observations
A

Metabolism:

  • 0.02% in liver

Physiologic effects:

  • ↑ HR ↓ SVR, ↓ BP, ↓ inotropy
  • Prolongs QTc, arrhythmogenic with epi
  • ↓ tidal volume, ↑ RR (minimal), ↓ central response to CO2 and hypoxia, bronchodilation

Characteristics/observations:

  • In the presence of dessicated soda lime CO is produced
  • Pungent odor

Q

Ultane/Sevoflurane

  • Metabolism
  • Physiologic effects (cardiovascular, respiratory, etc)
  • Characteristics/observations
A

Metabolism

  • 5-8% in liver

Physiologic effects:

  • ↔ HR, ↓ SVR, ↓ BP, ↓ inotropy
  • Prolongs QTc, arrhythmogenic with epi
  • ↓ tidal volume, ↑ RR (minimal), ↓ central response to CO2 and hypoxia, bronchodilation

Characteristics/observations:

  • In the presence of dessicated soda lime compound A is produced (especially in old absorbers)
  • < 2 MAC hours at FGF 1-2 L/min minimizes compound A in old absorbers
  • Non-pungent odor
