What forms the floor of the inguinal canal?
Ignuinal ligament - rolled inferior/free edge margin of aponeurosis of ext oblique. Pubic tubercle to ASIS
Lacunar ligament?
At pubic tubercle fibres of the inguinal ligament reflect laterally. Extend further laterally onto pectineal line to form the pectineal ligament
Where is there a defect in ext oblique?
Triangular ring superior lateral to pubic tubercle. Is the superficial inguinal ring.
Where is the SIR thickened?
Laterally and medially to form CRUS/crura that give rise to the external spermatic fascia
DIR?
Mid-inguinal point - half way between ASIS and pubic symphysis. 1.5-2cm above the inguinal ligament. Lateral to the inferior epigastric artery. Oval opening. Gives rise to the internal spermatic fascia of the spermatic cord
Inguinal canal…
Parallel to inguinal lig, passage runs obliquely through the anterior abdominal wall
Ant border?
Apon of external oblique and internal oblique at the lateral 1/3 (the lower fibres also curve over the canal to help form the roof. Then pass behind the canal and attach to the pubic tubercle - medially form the posterior wall of the canal.
Transversus abdominus?
Most deep. From ing ligament laterally. Some fibres curve posteriorly over the inguinal canal helping to form the roof. Medially also form the posterior wall.
Posterior wall of the canal.
Both trans abd and int oblique curve behind the canal and insert onto the pubic tubercle to form the conjoint tendon. Also transversalis fascia.
Contents?
Males: Spermatic cord and ilioinguinal nerve enters half way along to testis
Females: Round ligament of the uterus and nerve to labia majora
Spermatic cord contents
- testicular artery(para-aortic L2 - testes and epidymis)
- artery of vas def (vasectomy)
- cremasteric artery
- genital branch of genitofemoral nerve (L1, L2) - cremastor muscle and skin of scrotum
- **ilioinguinal nerve (L1) - pierces through int obl, runs outside of the cord and exits the SIR having not passed through the DIR
- sympathetic nerves from testicular plexus
- vas def
- lymphatics (para-aortic L2 nodes - origin of testicular arteries)
- tunica vaginalis
- pampiniform plexus (temp regulation, forms a single vein at DIR)
