glucocorticoids (not mineralocorticoids)
mechanism of GCs?
glucocorticoids bind to cytosolic glucocorticoid receptors. ligand-receptor complex translocates into the nucleus, binds to glucocorticoid response elements in promoter region of target genes, resulting up-regulation of anti-inflammatory gene expression. ligand-receptor complex can also interact with transcription factors to prevent transcription of proinflammatory genes. anti- inflammatory effect.
side effects of topical corticosteroids
restricted to the area of application. Hypopigmentation, hypertrichosis, skin atrophy, and telangiectasia are reversible side effects that can result from chronic use. Striae may also result but are irreversible
rare reaction to topical corticosteroids?
may result in systemic absorption of the glucocorticoid, causing symptoms of cortisol excess. from application of potent topical corticosteroids to large body surface areas (especially in infants and young children) or to occluded areas (e.g. diaper region).
what is the chosen topical corticosteroid vehicle?
ointment. exture of petroleum jelly. ointment formulation allows for better penetration of the active ingredient through the stratum corneum, increasing the efficacy. ointment form generally more potent than cream form for any given topical corticosteroid
