Diarrhoea (with blood and mucus), Weight loss, Abdominal pain, Malaise
Signs of Crohn’s disease
Apthous ulcers, Abdominal tenderness, Perianal abscesses, Fistulae, Clubbing, Erythema nodosum, Pyoderma gangrenosum, Conjunctivitis, Iritis
Investigations for Crohn’s disease
Bloods: FBC, U+Es, LFTs
Stool culture, Sigmoid/Colonoscopy
Blood findings with Crohn’s disease
FBC: Raised WCC, CRP - Low Hb
U+Es: Low K
LFTs: Low Alb
Management of a mild flare of Crohn’s disease
Prednisolone PO
Classification of a mild flare of Crohn’s disease
Systemically well
Management of a severe flare of Crohn’s disease
Hydrocortisone IV, Metronidazole PO or IV
Classification of a severe flare of Crohn’s disease
Systemically unwell
Long term management of Crohn’s disease
Azathioprine first line
Methotrexate, Biologics
Indications for surgery in Crohn’s
Obstruction, No response to drugs

Diarrhoea (blood and mucus), Abdominal pain (cramps), Weight loss, Tenesmus, Malaise
Signs of ulcerative colitis
Fever, Tachycardia, Abdominal mass, Clubbing, Erythema nodosum, Conjuctivitis, Iritis, Episcleritis, Arthritis
Investigations for ulcerative colitis
Bloods: FBC
Stool culture and microscopy, Sigmoid/Colonoscopy
Blood findings with ulcerative colitis
FBC: Raised CRP and WCC
Classification of a mild flare of ulcerative colitis
Less than 4 stools a day
