Q

Give an overview of the Giardia lifecycle

A

Q

What are the symptoms of Giardia infection?

A
  • Diarrhea
  • Steatorrhea
  • No fever
17
Q

What are clinical findings for Entamoeba infection?

A
  • 90% of patients are asymptomatic
  • diarrhea
  • chronic disease mimicking IBD
18
Q

What are imaging findings for Entamoeba infection?

A

Enlarged liver on CT and raised right hemidiaphragm.

19
Q

Briefly, what is the lifecycle of Entamoeba?

A

Mature cysts are ingested;
Enters GI tract, then skin, then liver, lungs, & brain;
Trophozoites and cysts passed in feces.

20
Q

What is the treatment for Entamoeba?

A

Metronidazole - kills trophozoites

Paromomycin/Loqoquinol - kills cysts

21
Q

What are the symptoms of cryptosporidium infection?

A
  • Abd cramping/pain
  • dehydration
  • nausea
  • vomiting
  • fever
  • weight loss
22
Q

Treatment of cryptosporidium?

A

Usually self-limiting.

Nitazoxanide is a new drug ~$3000/wk

23
Q

What is the toxin for C. diff?

A

Binary toxin –> A (enterotoxin) and B (cytotoxin)

24
Q

What are some risk factors for C. diff infection?

A
  • Hospitalization
  • Broad-spectrum Abo
  • PPIs (b/c they reduce effect of gastric acid in killing spores
  • Multiple co-morbidities
25
Q

Clinical Presentation of C. diff?

A

Diarrhea (watery, foul-smelling)
Increased WBC
Mild abd discomfort/pain

26
Q

What are some complications of C. diff infection?

A
  • Pseudomembranous colitis
  • Toxic megacolon
  • Intestinal perforation
  • Sepsis
  • Death
