EI IFSP
Family centered approach
- promotes family engagement in EI
- addresses developmental needs of child and family.
IFSP written plan- review IFSP documentation handout
summary statements of Child’s present level of functioning for:
- Physical health and nutrition
- hearing
- seeing
- moving
- communicating
- thinking and learning/cognition
- social skills and temperament
- Doing things by self/adaptive developmental performance.
IFSP plan must include:
1) family support and resource information
2) Priorities of the family, what are their needs or concerns for their child
3) family outcomes for the child (in their own words)
- goals, expectations for their child and family unit
- outcome statements
- who is the IFSP team?
EI service plan
List of services needed (include OT)
- where services will occur
- duration of service
- frequency and intensity of service
- method of service delivery (direct, indirect, consultative or combination of all)
Transition steps from IFSP to Preschool
Support child for services, if needed beyond EI services
-6 months prior to 3rd birthday must have meetings with local LEA.
(share information of child’s progress, determine if continuation of services is needed)
