Impella Study Flashcards
Which 2 Impella devices can be placed through an open chest?
Impella 5.5 and Impella LD
How is the Impella RP device placed?
Percutaneously through the left femoral artery and into the pulmonary artery
What size is the cannula on an Impella 2.5?
12 fr
What size is the cannula on an Impella CP?
14 fr
What size is the distal pigtail cannula on the Impella 5.0?
6 fr
What size is the cannula on the Impella 5.0 and Impella LD?
21 fr
Which two Impella devices do not have distal pigtails?
Impella 5.5 and Impella LD
What color are the right and left side pump plugs?
Blue right side
Red left side
What should be used as a purge fluid in the Impella?
D5W with Heparin, 25U/ml or 50 U/ml
What fluid should be avoided as a purse fluid?
Normal Saline
What is the lowest P level any Impella should be ran at?
P-2
Which display setting should be utilized during transport?
Placement setting to watch waveforms for proper placement
What kind of waveform should be present with the motor current, regardless of Impella type?
pulsatile waveform
Where are the inlet and outlet areas of the Impella RP?
Inlet in the IVC and outlet in the PA. Bypasses the right ventricle
What do you do when you receive a pump position wrong alarm?
You drop to P-2 level, begin supporting the patient medically, and alert the receiving
Are there position alarms for both left and right sided Impellas?
No. There are no position alarms for the Impella RP
What are the 3 main reasons for a suction alarm?
Low volume in left ventricle.
Incorrect pump position.
Right heart failure.
What do you do with a suction alarm?
Drop the P level by 1-2, reassess fluid status. Drop P level by 1-2 each time until suction alarm goes away. Do no go below P-2. If unable to resolve, notify receiving.
How do you troubleshoot a Purge Pressure high alarm?
Check purge fluid tubing for kinks. If not resolved, contact receiving.
How do you troubleshoot a Purge Pressure low alarm?
Check purge fluid tubing for leaks or loose connections. If not resolved, contact receiving.
How do you troubleshoot an Air in Purge Line Alarm?
Click the Purge Menu button and then De-Air Purge System. Follow the prompts.
Which pressure measurement do you use for patient care decisions and status?
MAP
Which P setting do you utilize during CPR?
P-2
What waveform needs to be assessed after achieving ROSC?
Motor current needs to be pulsatile. If not pulsatile, keep P level at P-2 and notify receiving. If it is pulsatile, increase P level to the previous level.
Which P level is required for defibrillation?
No requirement. Defibrillate at any time at any level.
Where should the head of bed be during transport?
No greater than 30 degrees.
What are you assessing for at the insertion site?
Hematoma and bleeding.
Should the Impella be flat against the skin at insertion site?
No. It should be ramped up at the angle of insertion. Flat against skin can cause bleeding.
Why are inotropes sometimes utilized with Impellas?
Impellas are preload dependent. The inotrope helps move fluid from the right side to the left side where the Impella sits.
Impellas are preload ______ and afterload ______.
preload dependent and afterload sensitive
What is the phone number for 24/7 Impella help?
1-800-422-8666
