- Methotrexate
- Sulfalazine
- Hydroxychloroquine
What is methotrexates mechanism of action?
- Binds competitively to DHFR
- Inhibits the synthesis of DNA, RNA and proteins
- Not a clear mechanism in non malignant diseases
What are the most important side effects of methotrexate?
- Mucositis - Pneumonitis, oral ulcers
- Hepatotoxic - Hepatitis, cirrhosis
- Bone marrow suppression
- Teratogenic, abortifacient
What investigations must you do before starting a patient on methotrexate?
CXR - causes ILD
FBC, U+E, LFT
How is methotrexate excreted?
Renally
What supplement can you give to reduce side effects of methotrexate?
Folic acid
What class of drug is sulphasalazine?
Aminosalicylate
How do aminosalicylates work?
They inhibit T cells and neutrophils
What are the main side effects of sulfasalazine?
- Rash
- Reduced sperm count
- Oral ulcers
- Myelosuppression
- Hepatitis
What are some side effects of hydroxychloroquine?
- Retinopathy
- Alopecia
- Sensitivity to sunlight
- NVD
What are side effects of all immunosuppressive drugs?
- Immunosuppression
- Bone marrow supression
- Increased risk of malignancy
- Hepatitis
What should you monitor in a patient taking hydroxychloroquine?
- Pre treatment eye screening
- Annual eye screening
