Tsacrolimus forms complexes with cytoplasmic immunophilin FKBP‑12, which blocks the action of calcineurin in activated T cells.
This prevents production of interleukin‑2 and other cytokines, which normally stimulate T cell proliferation and differentiation.

IL2 is the most important of these –> reduces T cell proliferation
Basiliximab
Anti-IL2 (CD25)
Binds and blocks IL-2 receptor alpha chain on activated T cells -> depletes them -> inhibits IL-2 induced T cell activation
Lower rates of rejection in quadruple therapy
ATG
Polyclonal anti-lymphocyte globulin
Blocks T cell membrane proteins (CD2, CD3, CD45)
