Hepatitis B (HepB)
Three doses: * Birth * 1-2 months * 6-18 months
Rotavirus: (RV)
Minimum age = 6 weeks Rotarix (RV-1) = 2 doses * 2 months * 4 months RotaTeq (RV-5) = 3 doses * 2 months * 4 months * 6 months
DTaP
* Diptheria, Tetanus, & Acellular pertusis * Daptacel or Infanrix * Ages 1. 2 months 2. 4 months 3. 6 months 4. 15-18 months 5. 4-6 years
TdaP
* Tetanus, Diptheria, & Acellular pertussis * Ages 7 and older * Boostrix - minimum age = 10 * Adacel - minimum age = 11 * One dose 1. 11-12 years
Haemophilus influenzae type b (Hib)
* Minimum age = 6 weeks * 4 doses 1. 2 months 2. 4 months 3. 6 months [not necessary if PRP-OMP (PedvaxHiB or Comvax) was given at 2 and 4 months] 4. 12-15 months (booster)
Pneumococcal Conjugate (PCV13)
* Minimum age = 6 weeks * 4 doses 1. 2 months 2. 4 months 3. 6 months 4. 12-15 months
Pneumococcal Polysaccharide (PPSV23)
* Minimum age = 2 years * Indicated for immunocompromised, immunosuppressed, and functional asplenics.
Inactivated Polio (IPV)
* Minimum age = 6 weeks * 4 doses 1. 2 months 2. 4 months 3. 6-18 months 4. 4-6 years
Influenza
Inactivated (IIV) * Minimum age = 6 months * Give annually until 2 years of age Live attenuated (LAIV) * Minimum age = 2 years * Give annually
MMR
Measles, Mumps, Rubella * Minimum age = 12 months * 2 doses 1. 12-15 months 2. 4-6 years
Varicella (VAR)
Chicken Pox * Minimum age = 12 months * 2 doses 1. 12-15 months 2. 4-6 years
Hepatitis A (HepA)
* Minimum age = 12 months * 2 doses * Given from 12-23 months * Separate the doses by 6-18 months
HPV
Human papillomavirus * Cervarix (HPV2) - female only * Gardasil (HPV4) - males and females * Minimum age = 9 years * 3 doses 1. 9 years 2. 1-2 months after 1st dose 3. 6 months after 1st dose
Meningococcal
N. meningitidis * Hib-MenCY - bivalent meningococcal with HiB * Minimum age = 6 weeks MCV4-D (Menactra) - Quadrivalent meningococcal * Minimum age = 9 months MCV4-CRM (Menveo) - Quadrivalent meningococcal * Minimum age = 2 years 2 doses * 11-12 years (can give earlier in high risk populations) * 16-18 (booster)

Vaccinations - 1 month
- HepB (2nd) [can be deferred to 2 month visit]
Vaccinations - 2 month
- HepB (2nd) [if deferred at 1 month] 2. Rotavirus (1st) 3. DTaP (1st) 4. HiB (1st) 5. Pneumococcal (1st) 6. Inactivated Polio (1st) To reduce needle sticks can give * Pentacel (DTaP + IPV + HiB) * Rotarix/RotaTeq (RV) * Prevnar13 (PCV13) * +/- Recombivax (HepB) If HepB needs to be given - can also use * Kinrix (DTaP + IPV) * Comvax (HiB + HepB) * Rotarix/RotaTeq (RV) * Prevnar13 (PCV13)

- RV (2nd) 2. DTaP (2nd) 3. HiB (2nd) 4. PCV13 (2nd) 5. IPV (2nd) To reduce needle sticks can use * Pentacel (DTaP + IPV + HiB) * Prevnar 13 (PCV13) * Rotarix / Rotateq (RV)
Vaccinations - 6 months
- HepB (3rd) (anytime 6mo-18mo) 2. RV (3rd) (if using RotaTeq RV-5) 3. DTaP (3rd) 4. HiB (3rd) (may not be necessary, see HiB card) 5. PCV13 (3rd) 6. IPV (3rd) (anytime 6mo-18mo) 7.
