What is ITP
Attack directed against platelets; Type II hypersensitivity
Causes of thrombocytopenia
Decreased production, Sequestration, Consumption, Destruction
Don’t make them, hide them, use them, destroy them
(Loss from bleeding is NOT usually a cause of thrombocytopenia)
Signalment of ITP (age, sex & breed)
Middle aged
Female > male
Cocker spaniel, Poodle, ANY
ITP historical finding include
Acute or Insidious Observation of bleeding Consequences of bleeding Site related signs Drug or vaccine history
ITP physical findings
Petechiae, Ecchymosis, Retinal Hemorrhage, Epitaxis, Melena, Pallor, Fever, Spenomegaly, Site related signs
Signs of anemia in ITP
Pallor, weakness, anorexia, tachypnea
Worse site of bleeding in ITP

ITP hematologic observations
Anemia (regenerative with time) Marked thrombocytopenia \+/- large platelets Hypoproteinemia Mild leukocytosis Absence of inclusions

Related to bleeding
TP, albumin, BUN
ITP urinalysis

ITP coagulation assay
Prolonged ACT
Normal PT and PTT

Treatment of ITP
Corticosteriods - high dose, slow taper Vincristine - low dose Cytotoxic drugs, IVIG Splenectomy Plasmapheresis Transfusion? \+/- tetracycline No need for vit K
