What are the Immediate Operator Actions for 1[2]-AOP-01.10 Pressurizer Pressure And Level?

Answer 1

• IF Pzr pressure <2300 & PORV open or leaking, THEN place in override and shut block valve.
• If channel failure is indicated, select non affected channel (Pressure, RRS, Level)
• IF pressure is <2275 psia or Ppress lowering uncontrollably, then verify spray valves closed.
• If two failed open, then place Spray Controller HIC-1100 in manual and operate Spray & Pzr Htrs as necessary.
• If one failed open, then select opposite Spray Valve, 1100 E (F).

Answer 2

• If Letdown lost – Stop CHG pumps and put switches in AUTO
• If CHG lost – Isolate Letdown by closing both [all three] valves

What are the Immediate Operator Actions for 1[2] – AOP-03.02 Shutdown Cooling?

Answer 3

• IF LPSI Pump amps NOT stable, then throttle closed Header Valves until amps stabilize
• IF pump amps do NOT stabilize, THEN STOP LPSI Pump
• VERIFY at least one LPSI pump operating.
  o IF not and estimated time to core boiling is less than 30 minutes, THEN INITIATE Containment evacuation and closure.

What are the Immediate Operator Actions for 1[2]-AOP-04.01 SFP Cooling?

Answer 4

• If water level requirement not met (23’ above irradiated fuel, 58’ 2 7/8”) – immediately suspend fuel movement in the pool and crane ops with loads. Restore level within 4 hours
• If boron concentration requirement not met (<1900 ppm) – immediately suspend fuel movement in the pool and initiate action to restore concentration > limit per 1(2)-NOP-04.04.

• If 2/4 RPS HI-POWER Pre-trip, TRIP Reactor, go to EOP-01

What are the Immediate Operator Actions for 1[2]-AOP-08.03 Steam Bypass Control System?

Answer 6

• If 2/4 RPS HI-POWER Pre-trip, TRIP Reactor, go to EOP-01
• If SBCS valve has failed open, then place Permissive Switch to Off.

• Verify SG level 60-70% and stable, if not take manual control of MFRV to control level 60-70% NR.
  (Note: a HPFRV and its associated LPFRV have flow-compensation logic such that operating one in manual with the other in auto may cause the valve in auto to reposition in at attempt to keep the feed flow rate constant)
• SG Level < 40% then Trip Rx and close 100% bypass if open
• SG NR Level > {80%} [75%] & affected WR is rising with Rx pwr >15% or LOL bypassed alarm clear - Trip Rx and Turbine, else only Trip Turbine

• SG NR Level < 40% – Trip Rx and Turbine
• SG NR Level > {80%} [75%] & affected WR is rising with Rx pwr >15% or LOL bypassed alarm clear - Trip Rx and Turbine, else only Trip Turbine

What are the Immediate Operator Actions for 1[2]-AOP-13.01 TCW?

• If a TCW pump is tripped, then ensure the other TCW pump is running, if the tripped pump is the only available pump, then send an operator to the breaker to verify the area is clear, announce over gaitronics and attempt one re-start.

• VERIFY CCW pump amps stable for running CCW Pumps, if not stable, STOP affected CCW Pump and place in PTL
• If CCW lost to N-Header, then isolate letdown by closing V2516, V2515, [V2522] and stop charging pumps. Place charging pump switches to Auto and operate per 1[2]-AOP-02.03.

What are the Immediate Operator Actions for 1[2]-AOP-50.07A/B Loss of DC Bus?

• If reactor trips, implement EOP-01 while continuing with procedure.
• If Ppzr < 2300 psia with PORVs in normal mode and if PORV open or leaking, then close by going to OVERRIDE and closing the Block valves if power is available
• If PORVs are in LTOP mode and RCS is solid with charging pump(s) running, the STOP all running charging pumps.
