What is Immediate Jeopardy (IJ)?
A situation in which the provider’s noncompliance with one or more requirements of participation has caused, or is likely to cause, serious injury, harm, impairment, or death to a resident.

When an IJ has been determined, when is the start date of the IJ?
The date that the entity’s noncompliance made serious injury, harm, impairment, or death occur or likely to occur.
When an IJ has been determined, when is the end date of the IJ?
The date that the IJ was removed. This happens when it is determined that all elements of the removal plan have been implemented or actions have taken were completed in a manner that eliminates further likelihood of serious injury, harm, impairment, or death. If the IJ was removed prior to the start of the survey, the surveyor should attempt to identify when the noncompliance was removed and enter that date as the end date.
What is a Singular Event?
One-time event in which one resident was harmed in an isolated incident that is not the result of a pattern or widespread issues.
What is considered a serious adverse outcome?
An outcome that results in, or likely to result in, 1 or more specific types of actual or likely Immediate Jeopardy.
