what is idiopathic orofacial pain?
unilateral or bilateral pain intraoral or facially in distributions of one or more branches of CN5 w/ unknown aetiology
how is idiopathic orofacial pain usually described?
persistent
moderate intensity
poorly localised
dull pressing or burning
how often and long does the pain occur?
daily
>=2hrs duration per day
goes on for >3months
are analgesics effective?
no - paracetamol, NSAIDs opiods
burning mouth syndrome - give 2 old names
oral dysesthesia
glossodynia - when only tongue affected
main symptoms of BMS - how long does pain last over how long? what areas of mouth is the pain felt? (3)
intraoral burning or abnormal sensation
pain recurs daily for >=2hrs, lasts more than 3 months
burning sensation felt superficially in mucosa - may be accompanied taste distrurbance
pain on tongue, palate, lips
what must be excluded prior to BMS diagnosis?
parafunctional habits
dry mouth
GORD
candidiasis
anaemia
haematinic deficiency
diabetes
thyroid dysfunction
medication
name 3 types of orofacial pain resembling primary headaches
migraines (w/ or w/o aura)
tension headaches
trigeminal autosomal cephalagias
migraine w/o aura - how many attacks? lasting for how long?
at least 5 attacks
4-72hrs
migraine w/o aura - what must headache have at least 2 of the following? at least 1 of what during the headache?
at least 2 of:
unilateral
pulsating
moderate - severe pain
aggravated by physical activity
at least 1 of:
nausea/vomiting
photophobia/phonophobia
migraine w/ aura - features
recurrent attacks
each attack lasts minutes
unilateral
fully reversible visual, sensory or other CNS symptoms - usually pre-empt headache arising
what is required for migraine w/ aura diagnosis?
at least 2 attacks with fully reversible aura symptoms:
visual
sensory
speech
motor
brainstem
retinal
