Inst or Hosp acquired pneumonia if in hops >48 hours, long term care >14 days, vent associated pneumonia
GNR (pseudomodas and acinetobacter), S. Aureus, Legionella, mixed
Extended spectrum cephalosporin or Carbepenem with antipseudomonal coverage
add amino glycoside or fluroquinolone to cover resistant pseudomonas until sensitivies come back
Pneumonia in critically ill or worsening over 24-48 hours on initial abc
MRSA
add Vanc or linezolid, boa den gram neg coverage
Bac meningitis in age <1 month
GBS, E.Coli/GNR, Listeria
Amp + Gent or Cefotaxime
Bac Meningitis 1-3 months
S. Penumo, N. Men, H. Flu
vanc + Ceftriaxone or Cefotaxime
Bac meningitis 3 months- adulthood
S. Pneumo, Meningococci
Vanc + Ceftriaxone of Cefotaxime
