When we are asked why patients are on ventilators, what do you say?
- for airway protection

What are the reasons for needing airway protection?
- GCS<8 and stroke patients
- Neuromuscular disorders
- Airway obstruction- tumours, inflammation or tumour
What are the reasons for needing ventilation?
- Hypoxia PaO2<8 kPa
- Hypercapnia PaCO2>6 KPa
- Haemodynamic instability
- Mechanism
When do we extubate?
- When we have sorted out the original problem that needed intubation
- Water free lung
- The patient is haemodynamically is stable
- No new infections above and below the diaphragm
- No new bronchospasm
What is the normal inspiration:expiration ratio in a normal patient?
- 1:3
- We usually have longer expiration but this swapped in a ventilated patient
Why is the inspiration longer in a ventilated patient?
-to recruit more alveoli and lungs and help with better oxygen transmission
In which patients should we cautious about auto PEEP or giving too much during inspiration?
-in asthmatic and COPD patients
What is flow acceleration?
-How quickly a breath is delivered
How many breaths can we deliver with a ET tube or tracheostomy?
90-150
How many breaths can we deliver with a mask?
30-40
What is flow trigger?
-it is the sensitivity of the ventilator that will be trigged according to gas flow to provide inspiration
What is the flow trigger in normal patients and asthma/COPD patients?
- Normal patients- 1L
